Manifested drug-related problems may contribute to hospital admissions of patients. Such hospital admissions, known as drug-related hospital admissions (DRA), represent a significant and potentially preventable burden on health systems. DRA often arise from the combination of multiple medications, making it important to pay increased attention to drug interactions, particularly in older patients who are taking a larger number of medications. DRA can result from inappropriate medication dosing or the selection of medications in the presence of chronic kidney disease. Drug-related problems can be either potential (possibly leading to real problems for the patient) or manifest (the problem already impacts the patient). This study aims to identify potential DDIs in the medication history of patients acutely admitted to University Hospital Hradec Králové via the department of emergency medicine. The objectives are to determine the prevalence of hospital admissions with at least one potential DDI in the medication history and characterize the identified potential DDIs with respect to their mechanism, severity rating, level of documentation, and potential consequences. However, a number of studies show that appropriate dosage adjustment is not provided in many cases. Our objective was to analyze appropriateness of drug dosages and contraindicated medications in patients with CKD admitted to the hospital in Hradec Králové via the department of emergency medicine. Today patients are more than ever informed about their disease and the available and appropriate treatment, due to the high volume of information that can be acquired through the Internet. The traditional role of pharmacists was the provision of medicines to the population. However, nowadays, this role has been changed so as to include other tasks, such as management of prescribed medicines, and promotion of health, since pharmacists can serve as a source of expertise knowledge to the customers.
